Job Duties
Responsible for the overall medical component for the care and treatment of terminally ill patients and their families
Reviews clinical information for each hospice patient and provides written certification that life expectancy is six months or less if illness runs its normal course.
Reviews each patient's clinical information before recertifying patient's terminal illness.
Educates practicing doctors and others engaged in health care services regarding the hospice and palliative care program and its potential benefits to patients.
Collaborates with the hospice Interdisciplinary Group (IDG) members to develop, provide and integrate the individualized written plan of care for patient and family.
Obtains privileges at one or more hospitals / nursing facilities which provide inpatient services to hospice program patients.
Establishes on-call physician rotation support - 24 hours a day, seven days a week.
Completes all job duties in a manner that meets the accepted standards of practice and the Evercare Hospice & Palliative Care Policies and Procedures.
Contributes to achieving the company's mission.
Works collaboratively with others to achieve goals.
Quality Improvement
Integrates QI results into the established plan of care. Participates in the quality assessment and performance improvement program.
Participates in hospice in-service trainings.
Follows accepted standards of practice for effective infection control and safety.
Professional Development
Participates as a preceptor in the orientation of new team members as assigned by the Executive Director.
Attends regularly scheduled in-services and educational conferences.
Develops and achieves professional growth goals and objectives and reviews with supervisor.
Affiliates with professional organizations that represent hospice and palliative care.

Experience Required:Qualifications:
Training and experience in the psychological and medical needs of the terminally ill.
Knowledge of palliative care with particular emphasis on control of symptoms associated with terminal illness.
Ability to work collaboratively with patient's attending physicians to implement the hospice program.
Ability to work collaboratively with hospice employees and volunteers as part of an interdisciplinary team.
License to practice medicine or osteopathy in the state of the hospice.
Board certification in a primary care specialty, a working knowledge in the principles of palliative medicine and symptom management.
Reliable transportation with appropriate license and insurance coverage.
